Community Care
Living at home for as long as possible is often a high priority for patients and families. While health care services provided through Ontario Health atHome can enable many patients to remain at home, additional assistive community services further enable independence and enjoyment of life and provide much-needed caregiver support.
Assistive community services include:
- Meal delivery and dining programs
- Assistance with daily activities
- Transportation services
- Caregiver support
- Friendly visits
People are encouraged to consider the benefits of other services available in the community. These services can help reduce the stresses of daily management for both the individual and the caregiver.
Ask your care coordinator about community care options available to you.
Community Nursing Clinics
Community nursing clinics provide services that address specific health care needs, such as IV therapy, wound care or rehabilitation therapy. Visits are arranged by your care coordinator and are by appointment only. Community clinics are staffed by specially trained nurses and are not the same as visiting a walk-in medical clinic.
Therapy & Specialized Services
There are a number of specialized services offered by Ontario Health atHome, as well as other community providers across Ontario, aimed at supporting patients with specific conditions or health care needs where the care provided may be more beneficial to receive in a community setting. Our care coordinators will work with you, your family and caregivers to determine the best care plan to ensure your health, safety and well-being.
School Health Support Services
School-aged children with complex health conditions and disabilities may be dependent on health care support to attend and be successful in a school setting. We provide these health care supports in-school to support integration.
