Ontario Health atHome is a Government of Ontario Crown agency with accountability to the Ministry of Health, Ministry of Long-Term Care and Ontario Health (opens in a new tab) . We are guided by provincial legislation, including Bill 175, Connecting People to Home and Community Care Act, 2020, Bill 135, Convenient Care at Home Act, 2023, Fixing Long-Term Care Act, 2021 (opens in a new tab) and the More Beds, Better Care Act, 2022 (opens in a new tab)
In accordance with government priorities to strengthen and better integrate Ontario’s health care system, we are working to modernize home and community care services within the Ontario Health Teams (opens in a new tab) model of care, so patients’ health care journeys are seamless and they receive better and faster care that meets their unique needs.
